Abstract: Health is the most precious values for human lives. Balanced diet, personal hygiene and regular exercise are very important to maintain good health. However, health can be affected by many factors such as age, genetic, nutrition, habits and environment. Hence, most of the factors are the main aspects that contribute to chronic disease in humans. Diseases are the major causes of death in the developed world. Disease is a change in function of human body considered to be abnormal or unhealthy. It can be characterized by various signs or symptoms. Therefore, Decision Support System (DSS) for health evaluation is developed in order to recognize the type of human disease based on the symptoms faced by people. In this research, Fuzzy Multi-Objective Linear Programming (FMOLP) approach as mathematical model is applied in this DSS.
